If you are unable to provide two (2) proofs of residential address in your name as outlined on the previous slides, you must comply with one of the following options in order to establish proof of residence:

Option 1:

Customers unable to provide proof of residential address in their name (as outlined on the previous slide) may have the person they reside with complete a Certification of Address form (flhsmv.gov/pdf/forms/71120.pdf) along with two proof of address documents as outlined on the form. The person the customer lives with must be present at the appointment or must sign the Certification of Address form in the presence of a notary. 

Note: This option is not available for Commercial Driver License (CDL) holders or applicants.

Option 2:

Customers who are homeless and reside in an agency where housing services are provided (such as a homeless shelter or probation home) may provide the following:

• a letter from an authorized Manatee County homeless shelter, transitional service provider or half-way house verifying the customers resides at the agency's address (must be dated within the last 60 days) AND
• Section A of the Certification of Address form completed by the customer, self-certifying their residential address.  

Customers who are homeless but do not reside in a housing agency may provide the following:

• Section A of the Certification of Address completed by the customer, self-certifying their residential address as "General Delivery"

Note: This option is not available for Commercial Driver License (CDL) holders or applicants.

STOP! Important Information! 

The Manatee County Tax Collector's Office only serves Manatee County residents.   

If you are a Manatee County resident (or becoming one today), go back to the previous question and make the appropriate selection.  

If you are not a Manatee County resident (or becoming one today), you cannot complete your transaction in our office.  Visit https://floridataxcollectors.com/your-tax-collector/ to find the Tax Collector's office nearest you.

By completing and signing this application, you understand that the issuance of a Florida Identification Card immediately voids/cancels all other Driver License and/or Identification cards issued from any US State or Territory (including Puerto Rico, US Virgin Islands, Guam, America Samoa and the Northern Mariana Islands). 

Once the transaction is complete, it cannot be reversed or voided.  If stopped by law enforcement while operating a vehicle after the issuance of a Florida Identification Card, a violator may be subject to some or all of the following:

1. Issuance of a citation
2. Arrest made by law enforcement
3. Seizure of vehicle by law enforcement
4. Other penalties

Voter Registration Application - By completing the Voter's Registration Application, you acknowledge that you understand you can provide your email address and receive sample ballots and other voting information from the Supervisor of Elections office by visiting votemanatee.com, calling 941.741.3823 or emailing info@votemanatee.com.

Your Voter's Registration Card will be issued and mailed by the Supervisor of Elections.  If you do not receive your card within two weeks, you must contact the Supervisor of Elections at 941.741.3823.

Be Prepared!  Florida Voter Registration applicants must be prepared to acknowledge the following statements during their transaction:

1. I affirm that I am a U.S Citizen.

2. I affirm that I have not been adjudicated mentally incapacitated with respect to voting or, if I have, my right to vote has been restored.

3. I affirm that I am not a convicted felon, or if I am, my right to vote has been restored.

Customers applying for Florida Voter Registration will also select their party affiliation during their transaction. 

Be Prepared!  Customers who decline to register to vote will be required to make one of the following selections during their transaction:

1. I do not wish to submit a new Florida Voter Application

2. I do not wish to update my existing Florida Voter Registration

3. I am not currently eligible to vote in the State of Florida

Residential Address Requirements for Commercial Driver License Holders

Commercial License applicants must present two proofs of Florida residential address in their name. A Certification of Address form can no longer be used.

Applicants must present either two proofs from List A* OR one proof from List A* and one proof from List B.

*Commercial License applicants who are unable to provide a proof of residential address from List A must provide a Declaration of Domicile filed with the Clerk of the Circuit Court pursuant to Florida Statues section 222.17 AND one proof of residential address document from List B.

Note: If you currently hold a Florida CDL and are unable to comply with the requirements indicated above, you may be eligible to change your address online.

Proof of Florida Residency Requirements for Commercial Driver License Holders

Effective October 7th, 2024, all customers applying for an original Commercial Learner's Permit (CLP) or Commercial Driver License (CDL) (including out-of-state transfers) must present one of the following documents to establish Florida residency:

IMPORTANT:  Your previous selection indicated that you are required to show a medical card during your transaction. The medical card must be completed in full by a qualified physician and must include their national registry ID #.  It must be legible and cannot contain any alterations, cross-outs, white-outs, or any other corrections.  

See www.fmcsa.dot.gov for additional medical card requirements.  If you are unable to provide an acceptable medical card during your appointment, you will be subject to our $6.25 service fee, you will be required to obtain a new card, and schedule a return appointment.    

By completing this application, you acknowledge that it is your responsibility to determine the appropriate selections throughout this application and whether an exception applies to you.  

Severe fines, penalties and incarceration are possible for violators who operate under an incorrect selection, do not maintain certification or provide any false/incorrect information to obtain a CDL license.  

We strongly suggest that you seek legal advice, the advice of your employer or consult the appropriate state and federal regulations.  Drivers are urged to visit flhsmv.gov and fmcsa.dot.gov for information and regulations. 

By completing this application, you acknowledge that you will review your driver license before leaving the office and inform the staff of any errors (endorsements, restrictions, spelling/address corrections, license types, etc.) so they can be corrected. 

Once you leave the office, the transaction cannot be voided or cancelled and corrections/changes will be subject to standard reissue fees. 

Appointments are required for all transactions.  After clicking submit on the next screen, you will be redirected to the Manatee County Tax Collector's website to schedule your appointment. 

Failure to select the correct transaction type will invalidate your appointment. Scheduling multiple appointments will result in all of your appointments being cancelled without notice.

Do not print this application; an associate will access your electronic submission during your appointment. Your application will be retained for 90 days. If you have not completed your transaction within this time, you must complete a new application.

At your appointment, be prepared to show the application completion email you will receive after submitting the application.
